If you are a registered parishioner of Saint Catherine Labouré Parish and wish to obtain a Sponsor Certificate for either the Sacrament of Baptism or the Sacrament of Confirmation, please click on this link.
- Complete the form, and hit Submit
- Print and sign the form
- Bring the form to the Parish Office for review and Pastor’s signature.
If needed, the parish will forward to the appropriate parish requiring the Sponsor Certificate.
Form must be submitted a minimum of two weeks prior to the date needed.
Sponsors from other parishes are asked to contact their own parish to obtain a sponsor letter OR may fill out the Sponsor Certificate Request form below, print, and take it to their own parish for approval.
Once approved, the certificate must be brought to St. Catherine Labouré Parish, 4000 Derry St., Harrisburg, PA 17111.
